OFFICIAL RULES
NO PURCHASE NECESSARY
1. HERE'S HOW IT WORKS: Dental practice employees can enter the dental office in which they work for a chance to win a Grand Prize of Levin Group's Total Practice Success Management Consulting Program for their dental practice (see Rule #4 for prize details). See Rule # 3 for details on how to enter the monthly drawings via mail or online. Grand Prize will be awarded to the dental practice indicated on each winning entry.
2. ELIGIBILITY: This promotion is open only to residents of the 50 United States (except Maine) or the District of Columbia who are 18 years of age or older at time of entry (except residents of AL & NE who must be 19 years or older at time of entry and residents of MS who must be 21 years of age or older at time of entry) and are employed at the dental practice indicated on the entry at time of entry. To be eligible, dental practice must be a licensed, independent General Practice (not affiliated with a dental practice management company), and must be located in the 50 United States or the District of Columbia. Employees of Procter & Gamble, its affiliates, subsidiaries, advertising, promotion and internet agencies and their immediate family members and/or those living in the same household of each are not eligible. All federal, state, and local laws and regulations apply. Void in Maine and where prohibited by law.
3. HERE'S HOW TO ENTER THE MONTHLY DRAWINGS:
• ONLINE: Beginning 8:00:00 AM ET on 10/1/2009, visit www.virtuallyplaquefree.com and follow the on-screen instructions to complete the entry form and submit an entry into the Sweepstakes. Entries must be received by 11:59:59 PM ET on 3/31/2010 to be eligible for the final Monthly Drawing.
• VIA MAIL: Complete an Official Entry Form, fold and seal it (no staples please) and mail the postage-paid entry form. Mail in entries will be counted towards the day they are received. Entries must be received by 3/31/2010 to be eligible for the final Monthly Drawing. No mechanically reproduced entries permitted. Not responsible for lost, late, illegible, mutilated, misdirected entries or entries not received in time for the random drawing.
Limit one entry per person per day, regardless of method of entry. If more than one entry is received per person per day, only the first such entry received will be eligible. Entries must be received by the deadline indicated in the chart below to be eligible for that Monthly Drawing only. Non-winning entries will not be rolled over to subsequent drawings.

Winning dental practices will be selected in a random drawing on or about each drawing date indicated in the chart above, from among all eligible entries received within the corresponding entry period indicated in the chart above. Random drawings will be conducted by D.L. Blair, Inc., an independent judging organization, whose decisions are final on all matters relating to this offer. Odds of winning in each drawing will depend upon the number of eligible entries received within the corresponding entry period for that drawing.
4. PRIZES: (12) Grand Prizes (2 per Monthly Drawing as indicated in the chart above): Each winning dental practice will receive Levin Group's Total Practice Success Management Consulting Program. The consulting program is tailored for General Practices and consists of three comprehensive phases over a 12-month period. The consultation program requires travel for the owner dentist of the dental practice to a Levin Advanced Learning Institute location in Baltimore or Phoenix for two days during Phases II and III and travel of a Levin Group Practice Consultant to the location of the practice for two days during Phase I. Winning dental practice will be responsible for travel expenses associated with the owner dentist travel to a Levin Advanced Learning Institute during Phases II and III. A travel stipend of $5,000 will be awarded to the dental practice in two equal installments (each as a check). (Approximate Retail Value: $35,900 per prize). All other expenses not specifically set forth herein are the sole responsibility of the winning dental practice. All terms and conditions of the Levin Group consulting agreement apply. Prizes consist of only those items specifically listed as part of the prize. Limit one prize per licensed dental practice.
5. GENERAL RULES: The owner of potential Grand Prize winning dental practices will be required to complete and return an Affidavit of Eligibility, Release of Liability, Prize Acceptance Form and, if legally permissible, a Publicity Release within 10 days of attempted delivery of same. In addition, the Owner of potential Grand Prize winning dental practices will need to attest that the entrant was an employee of the dental practice at time of entry. Noncompliance within this time period or return of any prize/prize notification as undeliverable will result in disqualification without further notice and an alternate winner will be selected. Winning dental practices will be notified by mail. Owner of Grand Prize winning dental practices will also need to sign the Levin Group consulting agreement document. Grand Prize winning dental practices must begin consultation program within 180 days of notification. No substitution or transfer of prize by winning dental practice permitted. Sponsor reserves the right to substitute prize of equal or greater value. All taxes and fees, if applicable, are the sole responsibility of winning dental practice. In no event will more than the stated number of prizes be awarded. By accepting prize, winning dental practices agree to hold Sponsor, Levin Group Inc., their respective directors, officers, employees and assigns, harmless against any and all claims and liability arising out of use of prize. Winning dental practices and those dental practice employees attending consultation program assume all liability for any injury or damage caused, or claimed to be caused, by participation in this promotion or use or redemption of any prize. Acceptance of prize constitutes permission for the Sponsor and its agencies to use winning dental practices' names and owners' names and/or likenesses for purposes of advertising and trade without further compensation, including a winners list, unless prohibited by law. Entrants agree to release, discharge and hold harmless Sponsor, Levin Group Inc., their respective directors, officers, employees and assigns, harmless from and against any and all liability and damages. By participating in this promotion, entrants agree to be bound by the Official Rules and the judges' decisions which are final. In the event there is a discrepancy or inconsistency between disclosures or other statements contained in any sweepstakes materials and the terms and conditions of the Official Rules, the Official Rules shall prevail, govern and control. All material submitted becomes the sole property of Sponsor and will not be returned. Sponsor not responsible for any typographical or other error in the printing of the offer, administration of the sweepstakes or in the announcement of the prizes.
NOTICE TO ONLINE ENTRANTS: ANY ATTEMPT BY AN INDIVIDUAL TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS PROMOTION IS A VIOLATION OF CRIMINAL AND CIVIL LAWS, AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH INDIVIDUAL TO THE FULLEST EXTENT PERMITTED BY LAW. Sponsor is not responsible for faulty, incorrect or mistranscribed phone transmissions, incorrect announcements of any kind, technical hardware or software failures of any kind including any injury or damage to any person's computer related to or resulting from participating in or experiencing any materials in connection with the promotion, lost or unavailable network connections, or failed, incomplete, garbled or delayed computer transmission that may limit a user's ability to participate in the promotion. Sponsor reserves the right to cancel or modify the promotion if fraud, misconduct or technical failures destroy the integrity of the program; or if a computer virus, bug, or other technical problem corrupts the administration or security of the program as determined by Sponsor/judging agency/administrator, in their sole discretion. In the event of termination, a notice will be posted online and the drawing for all remaining prizes will be conducted from among all eligible entries received prior to termination. Any damage made to the Web Site will be the responsibility of the authorized e-mail account holder of the e-mail address submitted at the time of entry. Proof of submitting entries will not be deemed to be proof of receipt by Sponsor. Any entries which are suspected of being fraudulent (including those using robotic, automatic, programmed or similar methods of participation) will be disqualified, based on determinations made solely by Sponsor. Sponsor reserves the right to prohibit the participation of an individual if fraud or tampering is suspected or if the individual fails to comply with any requirement of participation as stated herein or with any provision in these Official Rules.
6. WINNERS: Names of the Grand Prize winning dental practices will be posted online at www.virtuallyplaquefree.com periodically during the program, through 5/28/2010.
